Registration is now underway for the 2011-2012 school year.
To learn more about the school, or to obtain a registration packet, please call the school at (440) 248-8562 or email the school at soloncoop@yahoo.com.
Once you have a registration packet, the following instructions will assist you with the completion of the registration forms and include a brief description of how the data they contain impacts the operation of the preschool. These completed forms and the non-refundable registration fee of $75.00 are to be submitted at registration. Please make checks payable to “Solon Cooperative Preschool” or if paying by cash, please have exactly $75.00.
1. Registration Form: Please complete all spaces. Rosters and directories are compiled with the information from these forms. If there is a nickname for your child and you wish the child to be addressed by this name, please indicate it in the space provided. This name will be used by the Director and staff for identification purposes (i.e. name recognition).
On the back of the form is a space for your signature. Please be sure to sign and date the form. Below the signature line there are three areas for your initials. Please review these releases in detail and indicate your preferences.
2. Ohio Department of Job and Family Services Statement of Nonconviction: This form impacts our accreditation. Each participating member is required to complete this form due to our participation in the classroom with the children. If you need additional forms please let us know. Please complete only the top box. Please provide name, address, phone number, signature and date. A signature is also required at the bottom of the form as well (the last box on the form).
3. Membership Contract: This form outlines our responsibilities as Co-op members.
4. Member Committee & Position Designation: This form is collected by the Vice-President and is used to assign individuals to the committees and fill positions for the upcoming school year (Group chair is the only position required for Mom and Tot programs).
5. Tuition Payment Contract: This form outlines our financial responsibilities as Co-op members. Please indicate which type of installments you would prefer. Also, please sign both copies, return one and keep the other for your reference.
